This is not really an answer a medical scientist can give you, because this 
is more a question about what is real or what is not. This is because you can 
feel the same things in your dreams, as when you are awake; for example: 
fright, sexual arousal, grief, and other emotional states of mind. 
As far as I know, you can't feel any physical pain in your dreams, so you 
don't "feel" the broken leg (in your dreams), but your brain might let you 
think it, because it might seem that your leg is really broken. And as 
long as your mind thinks something is real, it will feel it too! But as 
soon as you think about it in a rational way, most of the time your 
brain/mind will filter out what is real or what is not. So to answer your 
question, you can never really tell if you're awake or if you are dreaming. 
This question has been keeping people busy over more then 5000 years, and 
the best answer I can give you is from an old philosopher who stated: 
"I think, therefore I Exist" -- Descartes
