One of just many problems that can occur. Impotence is the inability to maintain an erection. It is thought to be more a higher motor neuron issue, ie cerebral cortex. As an aside - our most important 'sex organ' is our mind. I imagine you are asking about erectile dysfunction. This term covers more of the physiology and anatomy. However, again, one must remember that sex/intimacy is much more than genitals. In terms of design, the anal sphincter system was not intended for objects entering. The wiring is all backwards. Thus, one can expect damage to occur, sometimes immediate, others as more of a repetitive damage pattern. Anatomically, there are two doorways. The internal circular door is controlled by the involuntary nervous system - it runs on automatic. When a bowel movement enters the rectum pressure is exerted against this 'door'. The door gradually opens while sending a message to you - 'time to find a bathroom'. The outer door is under conscious control - you release it at the appropriate time and place. When I do a digital rectal exam on a man I explain all of this to them. I also point out that doing a 'quick exam' causes a tremendous amount of pain as it violates the rule of function. Another big problem is rectal incontinence. After damage has occured, these doors start to lose their integrity both anatomically and neurologically. The wiring and the structure become damaged and nonfunctional. I am not sure what can be done at this time. Most surgical procedures where a rectum is created from bowel work best when the anal sphincter system is intact. A point I alluded to earlier covers the broader psychological aspects. One's sense of 'manhood' may be damaged in such an experience leading to conflicts inside over a sense of who they are and their 'value' as a person. Cases involving men raped in prison settings illustrate this most clearly. Finally a broad range of infectious diseases - from the more mundane VD to the exotic 'tropical bowel' or 'gay bowel syndrome' are the outcome many times. Along with this comes altered immune status which can culminate in AIDS, with HIV being implicated as the causative organism. Hopefully this helps.
