Check out the DifCo or similar microbiological medium manuals. Essentially, you autoclave the base medium with agar, allow the medium to cool, then add 5% blood while the base medium (with agar) is still liquid, then pour the plates. If you add blood to base agar that is too hot, it will break the red blood cells and you end up with "chocolate" agar.
