You should also remember that the acid normally secreted by the stomach is later neutralized in the jejunum by the secretion of bicarbonate ion. So, if less acid is available, less bicarbonate is secreted. It all balances out. I am not aware of any other sites where the PPI's act. I have been unable to find mention of any after a short search on Google. The PPI's only work when the pH is around 4. There are very few places in the body where that is the case. They irreversibly alter the H+/K+ ATPase in gastric parietal cells. The kidney is the only other organ I can think of that gets to that low a pH. The most common drug-induced acid-base problem is probably metabolic alkalosis caused by diuretic use. Regards; David.
